Chapter 1 - The Shadow Beneath the DoorThe emergency room doors burst open just after midnight, and my stepfather carried my twin sister inside as if he were rescuing her.

Emma’s head hung against his shoulder. I stumbled behind him, one hand pressed to my ribs, while my mother kept repeating the same sentence.

“They fell down the stairs. Both of them. It happened so fast.”

Wade set Emma on a gurney and turned to the nurse with a performance already prepared. His voice shook in all the right places. His eyes even filled with tears.

But Dr. Elena Ruiz did not look at him. She looked at us.

Emma and I were fifteen, identical except for the small scar above my eyebrow. That night, we also had identical bruises across our arms, shoulders, and backs. Same shape. Same spacing. Some dark and fresh, others yellowing at the edges.

Dr. Ruiz asked Wade and Mom to wait outside.

Wade smiled. “They are frightened. I should stay.”

“That was not a request,” she said.

A security guard stepped closer. Wade’s smile disappeared.

Once the door shut, Dr. Ruiz checked Emma’s pupils, then examined my arms. Her voice stayed calm, but her jaw tightened.

“Claire, did you fall down the stairs?”

I looked toward the door. Wade’s shadow remained beneath it.

Emma’s fingers twitched against the sheet. Dr. Ruiz leaned closer. “You are safe in this room.”

“No,” I whispered. “We are never safe in a room.”

The doctor froze. Then Emma opened her eyes and said the words Wade had warned us never to say.

“He lines us up so the bruises match.”

Dr. Ruiz stood, turned the lock, and pulled a silent alarm beneath the counter.

Outside, Wade began pounding on the door.

My mother started crying. “Please, you don’t understand. If the police come, he’ll tell them what happened to Daniel.”

Dr. Ruiz looked at her through the narrow window. “Who is Daniel?”

Mom pressed both hands to the glass. “Our daughters’ real father.”

The pounding stopped. For one terrible second, the entire hallway went silent.

Then Wade said through the door, very softly, “Karen, you just killed us all.”
